Students With Dietary Restrictions
HISD Food Services provides accommodations for all students with special diets. Please let us know if your child has any special dietary restrictions including any food allergies and Food Services will provide the necessary substitutions for them. A special diet request accompanied with a diet order signed by a licensed physician or recognized medical authority must be submitted. Print the Physicians Request for Special Dietary Accommodations linked below and have your licensed physician or recognized medical authority complete the form. Return the completed form to the school nurse and he/she will send the form to the HISD Food Services Nutritionist for review.

Requests must be submitted annually at the beginning of the school year or whenever a change in diet is required. Any necessary menu accommodations will be communicated by the nutritionist to the school nurse and cafeteria manager. Please allow several days for processing. It is recommended that you send a meal from home until the necessary accommodations are in place. Accommodations are considered only for those students requiring a change in diet based on medical need. Special dietary accommodations will not be made without the appropriate medical documentation.
